How to confirm coverage with your insurance
Each insurance plan has individualized coverage and there may be restrictions and/or limits on nutrition counseling benefits. It is your responsibility to call your insurance company to confirm coverage BEFORE scheduling an appointment. You will be responsible for any fees not covered by insurance.
To check your benefits, please call the member services number (usually located on the back of your insurance card) and ask the following questions. Thank you!
Self Pay Rates
If paying at or before the time of service and not using insurance, our rates are:
- Initial Appointment, up to 60 minutes: $250
- Follow-Up Session, up to 60 minutes: $175
Payment
A valid credit/debit card on file is required and will be used for self-pay fees, amounts insurance tells us are your responsibility (copays, deductibles, non-covered services), and late cancellation/no-show fees.
No-shows and late cancellations: 48 hours notice is required to cancel or reschedule your appointment. Less than 48 hours notice or failure to show up for your appointment will result in a $150 fee, charged to the card on file.
