We accept insurance!

Please review the following information regarding insurance and self-pay options before booking. It is your responsibility to ensure our services are covered by your insurance plan.

How to confirm coverage with your insurance

Each insurance plan has individualized coverage and there may be restrictions and/or limits on nutrition counseling benefits. It is your responsibility to call your insurance company to confirm coverage BEFORE scheduling an appointment. You will be responsible for any fees not covered by insurance. 

To check your benefits, please call the member services number (usually located on the back of your insurance card) and ask the following questions.  Thank you!

  • If you do not have coverage using those codes, ask if you have coverage for CPT codes 99401, 99402, 99403 and 99404.
  • Are there certain criteria that must be met to qualify (e.g. obesity or cardiovascular risk factors)?
  • Ask what ICD-10 codes are covered under your plan (e.g. overweight, obesity, IBS, dyslipidemia, hypertension, etc). 
  • Do I have a deductible to meet before insurance will pay? If so, how much is remaining to meet my deductible? When does my deductible renew?
  • Will I have a co-pay/co-insurance?
  • Note: If your plan does not cover ICD 10 code Z71.3, a referral or doctor’s note with a covered medical diagnosis will be required to submit the claim even if your insurance plan tells you it is not.
  • Some insurance plans may impose limits on the number of visits or units they are willing to cover.  This number can vary from 0 to unlimited depending on the carrier and medical need.

  • If not, ask if you have out-of-network benefits.
  • The cost of the appointment will be collected from you at the time of booking. After your appointment, we will provide you with a Super-bill that you can submit to your insurance company for possible reimbursement.
  • In-Network:  We will charge you for any deductible, co-pay, co-insurance, and non-covered services. The rest of your appointment will be paid to us directly from your insurance company.

Self Pay Rates

If paying at or before the time of service and not using insurance, our rates are: 

  • Initial Appointment, up to 60 minutes:  $250
  • Follow-Up Session, up to 60 minutes:  $175

Payment

A valid credit/debit card on file is required and will be used for self-pay fees, amounts insurance tells us are your responsibility (copays, deductibles, non-covered services), and late cancellation/no-show fees.

No-shows and late cancellations: 48 hours notice is required to cancel or reschedule your appointment. Less than 48 hours notice or failure to show up for your appointment will result in a $150 fee, charged to the card on file.
